Tag Instructions According to Maryland Transportation Code, Section 13-619, individuals desiring Organizational Plates must be a member in good standing of the non-profit organization. By signing the form, you are requesting membership in Choose Life of Maryland (CLM) and agree to pay annual membership dues of $15.00. Additionally a one-time fee of $25.00 is required by MVA to issue new plates, registration, and stickers. Instructions: 1. Complete and sign this application form. 2. Send the application form, along with a check or money order for $40.00 to the address on the left. Please make checks payable to: Choose Life of Maryland, Inc. 3. CLM will add you to their membership roster and send a MVA form back to you. 4. Complete the MVA form and return it to CLM. 5. You will receive your new plates, registration, and decals in 3-6 weeks. |
